The Support at Home Program The Support at Home Program The Support at Home Program is designed to support older individuals to maintain their independence and quality of life. The levels of support available in Aged Care include: The Support at Home Program An Aged Care Assessment Test (ACAT) assessor will determine if help is required. There are eight levels of support offered by Aged Care Australia: Level 1 : Minimal Support for Independent Living – Light housework, meal delivery, welfare check ins. Level 2 : Light Personal Care and Social Support – Assistance with dressing, showering, medications, social activities. Level 3 : Moderate Assistance with Daily Living – Regular personal care, mobility aid support, meal prep, household cleaning. Level 4 : High Frequency Personal and Domestic Support – Daily routine assistance, home modifications, toileting, continence care. Level 5: Daily Support with Health Monitoring – Hoists, transfers, transport to medical appointments, in home safety monitoring. Level 6: Comprehensive Health and Clinical Care – Nursing support, continence management, allied health therapies. Level 7: Intensive Daily Multidisciplinary Care – Assistance with all daily living activities, complex health conditions, dementia support. Level 8: Highest Classification of In Home and Palliative Care – Palliative support, 24/7 availability, specialised equipment, nursing interventions. Government funding is provided based on assessed needs. All elderly Australians can apply for a Support at Home Package regardless of income or assets and receive a fixed amount from the government. However, the co-payment will vary based on if you are a full pensioner, part pensioner and Commonwealth Seniors Health Card (CSHC) holder or a self-funded retiree and non-CSHC holder. It is important that you do not delay getting an assessment as there can be very long wait times from when you meet with an ACAT assessor to when you receive your package. Do you receive the money ? The government gives the money to a “provider” who holds the money. They pay companies like us for the care you receive. The provider will take a percentage of your package as an administration charge. Co-Payments As mentioned above, all Australians are able to apply for the Support at Home Program, however, there is a mandatory co-payment charged by the government. Depending on the services funded by your ACAT and your income, you may need to contribute to your support at home. The level of co-payment is determined by Aged Care Australia. The fees are NOT charged by us. These are the fees the government charges for using the package. This can be confusing for someone who is new to Aged Care and using government funding. Here’s a step-by-step guide to applying: 01. Check Your Eligibility To be eligible for the NDIS, you must meet the following criteria: Age Requirement – You must be under 65 years old when applying. Residency Requirement You must be: An Australian citizen, OR A Permanent Resident, OR A holder of a Protected Special Category Visa. Disability Requirement You must have a permanent and significant disability that: Affects your ability to take part in daily activities. Requires support from the NDIS. Requires assistive equipment or technology. Early Intervention Requirement (if applicable) If early support can reduce future disability impacts, you may be eligible (e.g., children with developmental delays). Evidence of Functional Impact How your disability affects your daily life. Your doctor or allied health professional may need to complete the NDIS Supporting Evidence Form 03. Wait for Assessment and Approval The NDIS will assess your application based on your disability, support needs, and documents. Create Your NDIS Plan Once approved: Meet with an NDIS Planner or Local Area Coordinator (LAC). Discuss your goals, support needs and funding allocation. Receive an NDIS Plan that outlines your funded supports. 06. Use Your NDIS Funding Choose how to manage your funding: self-managed, plan-managed, or NDIA-managed. It is important to remember that to receive government funding for Aged Care at home, you must apply through the My Aged Care program, which is managed by the Australian Government. Here’s a step-by-step guide to applying: 01. Check Eligibility You must be 65 years or older (or 50+ for Aboriginal and Torres Strait Islander people). You must need support to live at home independently. To check eligibility, you can: Call My Aged Care at 1800 200 422. 02. Get an Assessment An ACAT (Aged Care Assessment Team) assessor will determine if you qualify for the Support at Home Program and the level of financial support that will be provided. There are eight levels of financial care that may be awarded. 03. Apply for Services Once assessed, you will receive an approval letter and will be placed on a waiting list. For the Support at Home Program, you may wait several months depending on availability. 04. Choose a Provider Once you receive your Support at Home Program Package, there are many Providers you can choose to sign up with. Select a “Provider”.
